For example in this game the black bishop has a very poor value so i would not exchange it with my knight which is involved in my attack to king castle. By the way, black are not just offering an exchange, they are threatening at the same time e3 in order to recover the initiative

Well, normally I'd agree with you here 31415, since you're behind and exchanging will just make Black's position more comfortable. However, in this case accepting the exchange was absolutely the right thing to do, since it wins you a piece (and more) outright. 23...Qg5?? was a serious blunder: 24.Qxg5 hxg5 25.Rxb7, and now White has his pick of winning continuations.

I am baffled as to why Black didn't play ...Qxe3+ on move 24 (before ...Bc8), following White's mistake of not taking the Queen exchange (see my comment above).

You mean his e5 pawn. BTW, this does not win the pawn, if 28.Qd6, as the Queen is still defended and protects the e pawn.

Not a bad game. The thing is, however, that you don't want to go for tactics that are straightforward and easy to defend against. In most cases, this looses tempii, at the very least, and looses the game very quickly, at worst. I remember doing something like this, in terms of threatening f7, against a Class A Player. For a while, I made no mistakes, but that move turned no error into a loss in one. Always important to be aware of these things. In addition, It's important to be aware of your opponents strategies, and tactics because letting your guard down could mean disaster. Other then that, this game was completely flawless.

The thing is that 31415 wasn't going in for tactics that were straightforward. He was forcing black to weaken his position - by moving a pawn in front of his king, or by opening lines. So it didn't matter that black saw the threat here, because to do something about the threat, he had to weaken his position a bit. That's different to cheapo tactics that are easily rebuffed without creating any weaknesses at all.
