Wrong moveby black.
in this problem black moves from Qf5 to Qf4 which is absolutely wrong in my opinion. Why black shouldn't play Qf5 to Qf1 to MAKE #. hence the tactical problem itself is wrong one.

Yes. The notation, Qxf4, means that the queen took a piece that was on f4.

my last comment
my last comment was wrong because I completely forgot the obstacle of Bishop to the Queen's move pointed by me. i am sorry & do apologise.
